For us to best treat your spine condition or injury, we need to have a comprehensive understanding of your current health status, Please bring all of the items listed below when you come in for your initial visit or consultation. If we do not have all of your medical records and the films that are related to your current condition at the time of your arrival, your appointment may need to be rescheduled. Please bring the following items to your first appointment:

Insurance Coverage The physicians and extenders at the Spine Institute of Louisiana participate in many different insurance plans and are also Worker’s Compensation approved physicians. To access the list of plans in which our physicians participate, please click here. If you are a workers compensation patient, we must have approval for your visit as well as for any additional procedures the physician orders prior to treatment. While most of the physicians participate in many plans, an individual physician may not be on a specific plan. If you are not sure if your physician is a participating provider, please call us at 318.629.5555 or 318.629.5448. |
